Oligospermia refers to a sperm concentration below the World Health Organisation reference threshold of 16 million sperm per millilitre. Fewer sperm means a lower probability of fertilisation. Severe oligospermia (<5 million/mL) usually requires ICSI (directly injecting a single sperm into an egg), while mild-to-moderate cases may respond to IUI.
